Option 1

Fillable Word Application Form: Download,* bring up in Word,** fill in information, save with your last name as title, and email to address on form, along with your resume or cv if you have one.

*To download, click link and the file will open in a new window. Click on the download symbol at the top right of that screen, and it will go into your downloads folder.

**To open in Word, go into Word's 'file' menu and click open. Select the PDF, 'Website Application', and click to open.

You can then fill in the editable spaces and save with your last name. Then you're ready to email it to the address at the bottom of the form.

Option 2

Downloadable Application Form: Click on link and then click the download symbol on the new tab to download the pdf to your downloads folder. Print and fill out, before mailing or delivering printed completed application to address at the end of the application.
